The
“Smoke Away™ rebate is for the online purchase
of either the:
• Smoke Away Deluxe Kit - $40
• Smoke Away Premium Kit - $10
This rebate is obtained directly from Emerson Direct, Inc.
by completing the Smoke Away Rebate Offer form and following
the simple rules listed below.
To obtain the rebate:
• Purchase Smoke Away online from our designated web
site.
• Completely fill out all areas of this rebate form
and sign it.
• Submit the completed rebate form along with a copy
of the Smoke Away packing slip received with your order before July 31, 2005.
Smoke Away Rebate Limitations:
• Only Smoke Away kits purchased on this internet site between May 1, 2005
and June 30, 2005 are eligible for the rebate.
• Only one rebate form can be applied per kit. Please
submit each rebate form in a separate envelope in order
to ensure proper handling.
• Submissions for the rebate must be received by July 31, 2005.
• All rebates must be submitted by mail and include
a completed rebate form and a copy of the packing slip with
date of purchase.
• Emerson Direct employees, Ship Right employees,
resellers, distributors and affiliates are not eligible.
